 Submission Form Instructions

  1. Once you've been accepted to PNANS as a member, download and print our submission form.

  2. When filling out your submission form, please print clearly with an ink pen (No Pencil).

  3. Begin by filling in your membership number and the present date. Beside these blanks is a check box for Monthly Grading Specials. If your submission qualifies for the monthly grading special, please check yes.

  4. Fill out your personal information and if needed, seperate Ship To Information. You need only fill the Ship To section out if the submission is going somewhere other than the submitter's address.

  5. Next is the coin table. Each row represents a different coin to be submitted. Look in the first row labeled, (EX). This row is an example format for you to follow. Fill out the quantity of each coin being submitted. For example, if you're submitting (5) 2000-S Dime Proofs, write (5) into quantity. If you are submitting any other dimes besides 2000-S, use a different row to identify the coin. Be sure to write down the correct year and mint mark. Include the denomination in numeric form (EX. $.05, .10, .25, etc). Lastly, be sure to clain an insurance value for your coins. The insurance value should represent all quantities in each row. Do not over estimate the value of your coins. With regards to insurance, coin values do not always meet the value you claim.

  6. On the second page, total up the quantities of coins being submitted and total insurance values.

  7. Using your total coin count, and total insurance value, use the fee tables to determine grading and shipping fees. Fill in the blanks and calculate your total. Check the box for which form of payment you're using, and include your credit card number if needed.
